The Y is here to support you transitioning from the Prisma Health HeartLife program to independent wellness. YMCA Path to Success is a 6-week program designed to help you grow stronger, feel more confident and ease into a more active lifestyle after cardiac rehab. With workout and fitness support from a certified personal trainer twice a week, you will learn to build healthy habits that will last far beyond your time in the program.

Our goal is to help you take what you learn from your rehabilitation program as we help you build strength, confidence, and consistency - ultimately empowering you to exercise and be more active on your own!

There is a $150 registration fee, which includes a membership to the Y during the duration of the program.

After completing Path to Success and if you decide to stay with the Y,  we’ll waive your joining fee and work with you to find a membership option that fits your lifestyle and goals—so you can continue your wellness journey with confidence.

We also offer financial assistance.

Path to Success is a rolling enrollment, 6-week program designed to support you after graduating from Prisma Health HeartLife program. To get started, stop by the Life Center Y or complete this participant interest form >>

To provide the best care and a safe experience, we ask for the following prior to enrollment:

  • Intake Form from previous cardiac or pulmonary rehab, including:
    • Diagnosis or condition
    • Current functional mobility and any limitations
    • Any precautions or special considerations
    • Health History Form
    • Informed Consent
    • Medical Clearance (as needed)

Step 1: You’ll meet one-on-one with your trainer to complete paperwork, get an overview of the program, and share more about your health history and goals.

Step 2: We’ll then test how far you can walk for 6 minutes that we'll use as a baseline to help measure your progress over time.

Step 3: Twice per week, you’ll receive a 1-hour educational and hands-on training session led by a credentialed Y trainer in a safe, supportive environment.

Step 4: At the end of the program, your trainer will provide a personalized workout folder with exercises you can continue at the Y or at home.
